2. Once the form is complete, please call us to schedule your pets appointment, (952) 473-4631. There will be a booking fee of $50, which will be applied to your first visit. This fee is taken over the phone when scheduling the appointment. Appointments cancelled or rescheduled with less than 48 hours notice will not be eligible for a refund.

3. Email or bring your pets medical records with vaccine history. You can email them to wayzatapethospital@gmail.com.
